How to Register as a patient

To be eligible to register with us as a patient you need to be living within our Coventry catchment area - see map.

If you wish to join the practice please ask any member of the reception team for the appropriate forms. Please complete and return these forms to reception.

Bring in your NHS card or just come in to fill in a short form if you live within our practice area. You can also download and print the GMS1 form here or complete a pre-registration form online below.

Pre-Register with the Practice CLICK HERE - your request to join our list will be provisional upon confirmation of your address (within our practice catchment area), ID checks & completion of either the online Patient Health Questionnaire or the paper version HERE.

THIS ONLINE PRE-REGISTRATION IS NOT FOR PATIENTS ALREADY REGISTERED AND ATTENDING THE PRACTICE - ONLY FOR NEW POTENTIAL PATIENTS

Once you have filled in the online pre-registration this will be sent to our practice.  You will need to attend our practice to fill in a GMS1 registration form that the practice has to send to the Primary care Trust, and either bring in the New Patient Questionnaire that you have down loaded and filled in, or fill this in when you attend.

1NEW43 New Patient Health Questionnaire - Please print, complete and bring to the surgery. This includes an Alcohol Screening questionnaire as advised by the Department of Health.

We will not refuse an application on the basis of an applicant's race, colour, gender, age, social class, sexual orientation, appearance, disability or medical condition. We will refuse applications made by applicants who do not live within our practice area.

In the new GP contract, GP practices are not allowed to refuse patients who want to switch GP practice. This means that our practice cannot refuse to register patients that are already registered with another GP in our catchment area.

You will be registered with “The Practice” which means that you can see any of our doctors or other health practitioners. However you can state a preference of a “usual” doctor.  You may attend either of our two practices - Harnall Lane & Green Lane Medical Centres.

After registration you will be offered a health check appointment.

If you live outside the catchment area or move outside the catchment area you will need to find another GP - find local NHS services for your address HERE

When registering with the practice you will be asked to complete a GMS1 form, a health questionnaire, and to make an appointment with a Health Care Assistant for a Health Check. This will help provide basic information for the practice, until your patient notes arrive from your previous doctors (which can take 6 weeks).

Please fill in the health questionnaire form below, and then bring it when registering with the practice or at your first appointment with a GP or nurse. If you are unsure about any of the questions, please leave them blank and query with a receptionist or at your first appointment.
