Practice Audits
 1. Annual Review
In the annual review your practice reflects on the changes that have occurred during the last calendar year. These reflect changes in staff, workload and patient population.
The practice is unique in that normally there would be one large surgery with a smaller branch surgery. Most doctors would work at the main surgery and only occasionally at the branch surgery. Your practice has almost got the same population of patients at Green Lane Medical Centre and Harnall Lane Medical Centre. This means that doctors spend about 50% of the time at each surgery.
This audits shows the multicultural make up of the practice, as we document ethnic group, language and religion. We also show the number of patients in our practice, workload for the primary care team and prevalence of diseases.

2. Diabetes
The practice has been monitoring Diabetes care for many years. Dr Jetty has completed the Diabetes Diploma course at Warwick University and is the diabetes lead clinician for the practice. The Audit shows the care given by the practice to our patients and gives information about why blood tests are done and what the long term sugar level is that the practice is aiming for in order to optimise care for its diabetic patients. It also gives information about other necessary annual investigations for diabetic patients.

3. Coronary Heart Disease, Heart Failure, Atrial Fibrillation and Hypertension
Another area that has been audited in the long term. The audit gives information about prevalence (existing number of patients with the disease), blood test levels, medication and other investigations. Dr Kapma is the lead clinician in this area for the practice.

4. Asthma and Chronic obstructive Airway disease (COPD)
Prevalence of both diseases is on the increase in Coventry. Smoking is a major contributor to the development of COPD and is a rare condition in individuals who have not smoked. The audits show the prevalence, investigations and treatment of the diseases. Dr Matthews is the lead clinician for the practice.

5. Stroke/TIA
The audit shows prevalence, the necessity for certain investigations and target levels. The practice aims to keep the prevalence of stroke as low as possible. Dr Jackson is the clinical lead for this audit.

6. Epilepsy
The audit shows prevalence, the necessity for certain investigations and target levels. Dr Jackson is the clinical lead for this audit.

7. Primary Prevention
Prevention of diseases is a very important area of our work. Preventing diseases before they start is always better than treatment. This audit shows data in the 30 to 74 years age group. This age group has been chosen as the 5 and 10 year Heart disease and Stroke risk score can only be calculated in this age group.
